Application and Online Registering

Create an online account within Credentia Platform.  The Credentia Platform makes online registration quick and easy, and much faster than mailing a paper application.

Account Creation

To create a Credentia account, follow the steps below:

1. Navigate to http://www.credentia.com/test-takers/dc.
2. Click Login.
3. Click Sign-Up to create new account.
4. Enter the required details.
5. Select the check box corresponding to By clicking Sign Up, you agree to our Terms and Data Policy.
6. Click Sign Up to create a new account.

Registration Process

• Once your profile account is created, you can create an "application to test." Select your eligibility route, complete the form uploading all required documents, and submit for approval. 
• Once you are approved to test you can schedule your exam and pay for the exam by credit card or pre-paid credit card (American Express, MasterCard, Visa, or electronic voucher). Fees are non-refundable and non-transferable once you’ve paid. 
• For exams at test center locations, you must make an online reservation via a laptop or computer (smart phones and tablets are not recommended) at least two (2) calendar days prior to the test date (not including the test date).
• For Written or Oral Online exams, you can schedule an exam as early as one (1) day following registration, depending on exam availability.
• Complete your online application in the Credentia Platform also you can ask someone from your nurse aide training program or facility employer for help in completing the application.
